Registration & Forms
Thank you for joining the Saint Paul the Apostle school family!
Below are details and forms to help the registration process. Registration will be held Wednesday, July 20 from 5:30 to 7:30 and Sunday, July 31 from 9AM - 1PM, both in Denning Hall.
Each family should receive a letter by U.S. Mail in late
June. You will receive a student information sheet for each child
enrolled with this mailing. Please check the information and add emergency
contact info. Initial in the top right hand corner if everything is
correct.
Please note that the Parent Handbook is available for you to print from
the website from the K-8 Parent Handbook tab. There will be a sign off
sheet for you to sign at registration.

Permission to Administer Medication
To provide medication at school if you would like your child to be able to request ibuprofen or acetaminophen, or if your child needs to take a prescription medication while at school. One form required per student.

Certificate of Immunization
New immunization forms plus a physical form should be filed each year for Preschoolers. Kindergarten needs a new immunization certificate. If you are not new or in Preschool or Kindergarten, immunization certificates should already be on file from last year.

Blood Lead Test
Parents must show that children entering Kindergarten have been tested for dangerous levels of lead in their blood. For more information on childhood lead poisoning prevention and testing, visit:

Dental Disease Screening
Parents of children entering Kindergarten must also show they have received a screening for dental disease. Dental Screening Certificate.pdf
